What does it do?

Adult circumcision is a simple operation to adult patients that involves the surgical removal of the foreskin on the penis. It is usually performed under anaesthetic injection, so that you remain comfortable during the procedure.

Tight foreskin (phimosis), repeated infections or swelling in the foreskin, or if the foreskin is not being able to be pulled back properly are medical conditions why some men may opt for circumcision. There are still those who do so for personal, cultural reasons or religious motives.

Health conditions in adults that may need circumcision

Tight foreskin (Phimosis)

When the foreskin is too tight, it is hard or impossible to pull back over the head of the penis. In appearance, it is a tight ring around the tip of the penis.

Repeated balanitis

Balanitis occurs when the skin of the penis and its foreskin get red, swollen, and infected. Most commonly, it is a yeast infection, but bacteria, viruses, or skin issues can also be a trigger.

Paraphimosis

This is when your foreskin is pulled up and gets stuck that way. It causes swelling pain. Because it can block the blood from flowing past the tip, it needs quick treatment.

Balanitis xerotica obliterans

This is a condition that makes your foreskin stiff and can cause learning on your penis end. It can lead to problems such as phimosis.

Penile cancer

The procedure
The Circumcision Procedure
How is it done?
  • You will get local anaesthetic to numb the area during the procedure.
  • The doctor adjusts and measures the foreskin to determine how much to cut.
  • The marked skin is then incised with a scalpel.
  • The doctor immediately stops all bleeding.
  • The skin is sewed back together with dissolvable sutures that disappear on their own.
  • The area is wrapped in order to protect it as well as assist in healing.
  • You will rest a bit, then be back home the same day if everything is well.

What are the different techniques used?

Adult circumcision surgery can be performed in a few different ways. Which option your doctor will use will depend on your specific needs and the one that will be most effective for your body. Here are some of them:

Dorsal slit technique

This is a process of cutting the top portion of the foreskin straight across below before removing it.

  • Best for: Men who have a very tight foreskin
  • Why it is used: It is simple, fast and effective if the foreskin cannot be retracted.

Sleeve technique

In this procedure the doctor cuts off a ring of foreskin, similar to removing an arm of a sweater. The skin is sutured cleanly afterwards.

  • Best for: Men who have thicker skin or more veins under their foreskin.
  • Why it is used: It has more control over bleeding and might appear more even when healed.
Pain Management
Is it painful?

You should not be in pain as anaesthesia is applied and there might be some soreness afterwards, but the procedure itself should be completely pain free.

Surgery Duration
How long does the procedure take?

After the anaesthetic takes effect, it only takes a few minutes to perform the circumcision. The entire process is roughly an hour and a half at the clinic for prep work, the procedure and 15 or so minutes of recovery before you can head home.

When can I return to work after the procedure?
If you have a desk job, you can generally return to work within 2 to 3 days. But if your work requires heavy lifting or a lot of physical activity, you are better off resting at least five to seven days. Allowing your body time to recover will speed your healing time and prevent complications.

The essentials

  • Clean your genitals thoroughly before your visit.
  • Wear light clothing and loose underwear.
  • Have someone you trust drive you home.

  • Your penis may be swollen and bruised for one to two weeks. If necessary, you can take pain killers.
  • After the surgery, you may eat as usual. A little nausea in the first few hours is common with anesthesia. Begin with clear fluids and light foods.
  • For the first three days, relax but take brief walks every couple hours. Don’t lift anything heavy or perform activities that put pressure on the area for 6 weeks.
  • Apply ice for 15 minute intervals several times per day, for the first 2 to 3 days, to minimise pain and swelling.
  • Refrain from sexual activity and erections for 4 to 6 weeks.
  • If you experience any bleeding, apply pressure for 10 minutes.
  • Stitches should dissolve in 2 weeks. Do not be alarmed if you notice a little redness or bruising.
  • Shower after 2 days but do not bathe or swim for 4 weeks. Gently wash around the area.
  • If necessary take the painkillers and stool softeners the doctor prescribes.
  • Do not take aspirin or ibuprofen for 3 days.

  • Your pain is not getting better even after you took your medicine.
  • Stitches seem loose or the wound begins to open.
  • You see that your bandage is soaked with bright red blood.
  • You observe signs of infection such as:
    • More swelling, heat, or redness in or near your wound area
    • Red lines on the skin around the wound
    • These signs include pus or foul smelling discharge.
    • A fever or chills
  • You cannot urinate or it hurts when you urinate.
  • You notice blood in your urine, or if you have pain in your lower back or sides.
  • You are too sick to eat or drink normally.
  • You experience pain, swelling or redness in your leg, especially if it is in your calf or thigh, which could indicate a blood clot.

The risks
Can a risk be prevented?

Although circumcision in adults is generally very safe, there are risks as well as potential side effects:

  • Bleeding and infection
  • Swelling and soreness
  • The healing site may appear uneven or unsightly after a few days.
  • Some men may notice that the tip of their penis becomes less sensitive, particularly during sex.
  • Sensitivity in the area of the scar.

Less common causes can be connected to the actual foreskin, including:

  • Removing excessive or inadequate foreskin.
  • It fails to heal as it should.
